Q. What is the Procedure of Registration of Birth & Death ?

Every Birth or Death is to be reported and registered within 21 days at the place of its occurrence in the prescribed reporting forms.

Form No. 1 : For Birth Registration

Form No. 2 : For Death Registration

Form No. 3 : For Birth Information

Form No. 4 : For Death Information

Form No. 5 : For Name Entry Information

The persons required to register Birth / Death are:

(a) When the Birth / Death has taken place in a house. Head of the house or nearest relative or oldest person in the family
(b) When the Birth/Death takes place outside the house:
(i) In a Hospital/Health Centre/ Maternity/Nursing Home or other like institutions Medical Officer Incharge or any officer authorised by him.
(ii) In a Jail Jail Incharge/Jail Superintendent
(iii) In a Hotel, Dharmshala, Boarding House etc. Person Incharge/Manager
(iv) In a Moving Vehicle Person Incharge of the Vehicle
(v) Found deserted in a public place Headman of the Village/Incharge of the local police station

Q. What is the Time Limit for Registration?

  • Name Enter within one Year - fees Rs.0/-.
  • Name Enter After one Year - fees Rs.10/-.
  • Within 21 days from the date of occurrence of live Birth / Still Birth / Death. - Without late fee
  • Above 21 days but less than 30 days - with a late fee of Rs.5/-
  • With orders of Distt. Registrar - above 30 days but within one year- With a late fee of Rs.10/-
  • With orders of Judicial Magistrate or a Metropolitan Magistrate/SDM - after one year. With a late fee of Rs.20/-

Q. Which papers/documents/fees, do I take with me?

(1) Application for Birth/Death Registration in prescribed Performa (downloaded).

(2) Proof of Birth/Death of the person in respect of whom certificate is required.

(3) Affidavit/Self Declaration specifying place, date and time of birth/death of the person.

  • Birth Certificate
  • Death Certificate
  • Copy of Ration Card.
  • School leaving certificate, if any, showing date of birth (in the case of birth certificate only).All documents should be attested.
  • All documents to be attested by a Gazetted Officer.

You should be getting a response normally within 5 working days


Registration of Births and Deaths Act, 1969

Civil registration is the act (process) of registering births and deaths in a country to capture data on the population trends. This is done for easier planning of government departments in rendering services to the public and to establish causes of death for public health department.
